Prescription Service Eligibility

Prescription medications are available only to patients who meet the medical criteria established by our licensed healthcare providers.

To receive a prescription through Dose Aesthetics, you must:

  • Be at least 18 years old or have the consent of a parent or legal guardian where permitted by law.
  • Be physically located in the State of Ohio at the time of your medical evaluation or telehealth consultation, unless otherwise permitted by applicable law.
  • Provide complete, accurate, and truthful medical information during your consultation.
  • Complete any required medical evaluation before a prescription can be considered.

Receiving a consultation does not guarantee that a prescription will be issued. Our providers use their independent medical judgment to determine whether treatment is appropriate and reserve the right to decline prescribing medications when clinically indicated.

Medical Evaluation Requirements

Before prescribing any medication, we require a medical evaluation conducted by one of our licensed healthcare providers.

Your evaluation may include:

  • A review of your medical history
  • Current medications and supplements
  • Allergies
  • Existing medical conditions
  • Symptoms and treatment goals
  • Additional questions or diagnostic information when necessary

Depending on your individual circumstances, we may request laboratory testing, medical records, or an in-person examination before making treatment recommendations.

We may decline treatment if we determine that a prescription is not medically appropriate or if additional evaluation is necessary.

Telehealth Services

When available, we offer telehealth consultations in accordance with applicable federal and Ohio laws.

By participating in a telehealth appointment, you acknowledge that:

  • Telehealth has limitations compared to an in-person examination.
  • Certain conditions may require an in-person visit before treatment or prescription services can be provided.
  • Technical issues may interrupt or delay your consultation.
  • You are responsible for ensuring that you participate in your telehealth visit from a private location with a secure internet connection.

If our provider determines that telehealth is not appropriate for your medical needs, we may recommend scheduling an in-person appointment or seeking emergency medical care if necessary.

Patient Responsibilities

To help us provide safe and effective care, you agree to:

  • Provide accurate, complete, and up-to-date health information.
  • Notify us of any changes to your medical history or medications.
  • Follow all prescribed treatment instructions.
  • Use prescribed medications only as directed.
  • Attend scheduled follow-up appointments when recommended.
  • Contact us promptly if you experience side effects or complications.

Providing false or misleading information may result in delayed treatment, denial of prescription services, or discontinuation of care.

Refund and Cancellation Policy

Medical consultations, provider evaluations, and telehealth appointments involve reserved clinical time.

Unless otherwise stated:

  • Appointment cancellations or rescheduling requests should be made at least 24 hours before your scheduled appointment.
  • Missed appointments or late cancellations may be subject to applicable fees.
  • Consultation fees are generally non-refundable once services have been provided.
  • Prescription medications, once prescribed, dispensed, or shipped, cannot be returned or refunded except where required by applicable law.
  • If a provider determines that treatment is not medically appropriate after your evaluation, consultation fees may still apply.

Refund requests for non-medication services will be reviewed on a case-by-case basis in accordance with our office policies.

Jurisdictional Limitations

Our prescription and telehealth services are intended for patients who are legally eligible to receive care under applicable laws.

Prescription services are available only where our healthcare providers are authorized to practice and prescribe medications.

We reserve the right to refuse services if:

  • You are located outside an area where we are legally authorized to provide care.
  • Applicable laws prohibit the requested treatment.
  • We are unable to verify your identity or eligibility.

Prescription Fulfillment Process

If our provider determines that prescription treatment is medically appropriate:

  • Your prescription will be issued by a licensed healthcare provider.
  • The prescription will be transmitted to your selected pharmacy or our designated pharmacy partner, as applicable.
  • Pharmacy processing times may vary and are outside our control.
  • Insurance coverage, prior authorization requirements, and medication availability are determined by your pharmacy and insurance provider.
  • We cannot guarantee medication availability or delivery timelines.

Patients are responsible for reviewing all medication instructions, warnings, and educational materials provided by the pharmacy before beginning treatment.

No Guarantee of Results

Every patient is unique, and treatment outcomes vary based on individual health factors, medical history, and adherence to treatment recommendations. We do not guarantee any specific medical, aesthetic, or therapeutic outcome.

Changes to These Terms

We may update these Terms & Conditions from time to time to reflect changes in our services, policies, or applicable laws. Updates will become effective when posted on this website. Your continued use of our website or services constitutes your acceptance of the revised Terms & Conditions.
